Late Arrivals: Late arrivals may result in a shortened appointment or rescheduling.
3. Payment:

Payment Due: Payment is due at the time of service unless you make prior arrangements.
Insurance: We will assist in filing insurance claims; however, patients are responsible for any balance not covered by insurance.

Limitation of Liability: Vitalix PolyClinic is not liable for any indirect, incidental, or consequential damages arising from dental treatment.
Emergency Care: In the event of a medical emergency, we will provide necessary care and may transfer patients to a hospital or emergency facility.
